Please have three recommendation forms filled out, signed, and returned by your references. If you are currently working, you should have one from your supervisor who is able to comment on your technical competence and other qualities, and two from professors that know you from your undergraduate or other academic programs. |
Please identify your academic advisor as soon as possible in the first semester, and fill out this form. This downloadable version is for you to use as a draft. You can pick up the form from the department after you and your academic advisor agree on your program of study. For this purpose, you also need to fill out the First Semester Advising form (see below).

To become acquainted with faculty members in your area of specialty and choose from them your Academic Advisor, you are asked to meet with all professors in your area so you will learn about (1) the courses that they teach that may be of interest to you, (2) their teaching plans, (3) their research interests, and (4) potential opportunities to work with them as a research assistant. Within your first semester, please make an appointment and meet with each of the faculty member in your specialty area as identified below. Bring this form with you to all meetings and ask each professor to sign after the meeting. This form must be returned to the Graduate Program Director before you are allowed to register for classes in the following semester. A Graduate Program of Study form also needs to be filled out and approved by your Academic Advisor, the Graduate Program Director, and the Chair.
